Mechanism of Action

The formulation combines three active agents that target pain pathways via complementary mechanisms:

  • Caffeine (CNS Stimulant & Vasoconstrictor): Acts as an adenosine receptor antagonist. It causes mild cranial vasoconstriction (counteracting cerebral vessel dilation associated with vascular headaches) and enhances the absorption and analgesic efficacy of both acetaminophen and aspirin.

Indications and Dosing Guidelines

This medication is indicated for the temporary relief of pain associated with tension headaches, migraine attacks, muscular aches, arthritis, sinusitis, toothaches, and menstrual cramps.

  • Adults and Children 12 Years and Older:
  • Take 2 caplets every 6 hours with a full glass of water while symptoms persist.
  • Maximum Daily Limit: Do not exceed 8 caplets (2,000 mg Acetaminophen / 2,000 mg Aspirin / 520 mg Caffeine) within a 24-hour period unless directed by a physician.
  • Children Under 12 Years: Do not administer without explicit medical guidance.

Side Effects

Common Adverse Effects

  • Gastrointestinal: Heartburn, dyspepsia, stomach discomfort, or mild nausea.

Serious Adverse Effects

Critical Warnings and Precautions

Severe Liver Damage Warning (Acetaminophen)

Acetaminophen can cause severe or fatal liver injury if you:

  • Take it concurrently with other medications containing acetaminophen (prescription pain relievers, cough/cold syrups).

Stomach Bleeding Warning (Aspirin)

As an NSAID, aspirin significantly increases the risk of severe stomach bleeding. The risk is elevated if you:

  • Are age 60 or older.

  • Take blood thinners (anticoagulants like warfarin or apixaban) or systemic steroids.

Reye’s Syndrome Warning (Pediatric Contraindication)

  • Do NOT give this medication to children or teenagers recovering from viral illnesses, such as chickenpox or flu-like symptoms. Aspirin is strongly associated with Reye’s syndrome, a rare but life-threatening condition characterized by acute encephalopathy and fatty liver failure.

Medication Overuse Headache (Rebound Headaches)

Using combination headache remedies frequently (more than 2 to 3 days per week on a regular basis) can induce medication overuse headaches (MOH), leading to a chronic daily cycle of rebound head pain.

Pregnancy & Lactation

  • Pregnancy: Aspirin is not recommended during pregnancy, particularly in the third trimester (after 20–30 weeks), as it can cause premature closure of the fetal ductus arteriosus, low amniotic fluid (oligohydramnios), or maternal/fetal bleeding complications during delivery.
