Mechanism of Action

Gabapentin is a structural analogue of the inhibitory neurotransmitter aminobutyric acid (GABA), but it does not bind directly to receptors, nor does it inhibit GABA uptake or degradation.

  • Subunit Binding: Gabapentin binds with high affinity to the auxiliary subunit of voltage-gated calcium channels (VGCCs) located presynaptically in the central nervous system.
  • Reduction of Excitatory Neurotransmitters: By modulating calcium channel trafficking and reducing calcium influx into presynaptic terminals, it attenuates the release of excitatory neurotransmitters such as glutamate, substance P, and calcitonin gene-related peptide (CGRP).
  • Neuronal Stabilization: This reduction in neurotransmission suppresses hyperexcitable neuronal circuits involved in central sensitization, seizure propagation, and neuropathic pain pathways.

Indications & Dosage

Approved Indications

  • Postherpetic Neuralgia (PHN): Management of persistent nerve pain following herpes zoster (shingles) in adults.
  • Partial-Onset Seizures: Adjunctive therapy in adults and pediatric patients 3 years of age with epilepsy.
  • Common Off-Label Uses: Diabetic peripheral neuropathy, restless legs syndrome (RLS), fibromyalgia, generalized anxiety disorder, and alcohol withdrawal management.

General Adult Dosing

  • Postherpetic Neuralgia Titration:
  • Day 1: 300 mg once daily.
  • Day 2: 600 mg/day (300 mg twice daily).
  • Day 3: 900 mg/day (300 mg three times daily).
  • Maintenance: May be titrated up to 1,800 mg/day (600 mg three times daily) based on efficacy and tolerability.
  • Epilepsy Maintenance: 900 mg to 2,400 mg daily administered in three equally divided doses. The maximum dosing interval must not exceed 12 hours to maintain seizure coverage.
  • Renal Dose Adjustment: Because gabapentin is eliminated exclusively via renal excretion unchanged, dosage must be reduced in patients with a creatinine clearance under 60 min
  • Administration: Swallow the capsule whole with water. Do not open, chew, or crush. It can be taken with or without food.

Side Effects

Common Adverse Effects

  • Drowsiness (somnolence) and fatigue
  • Dizziness and unsteadiness
  • Ataxia and poor coordination
  • Peripheral edema (swelling in the lower extremities)
  • Tremor, dry mouth, and blurred or double vision (diplopia)

Severe Reactions

  • Respiratory Depression: Severe, shallow, or labored breathing, particularly when combined with opioids or in patients with underlying lung disease.
  • Suicidal Ideation and Behavior: Increased risk of depression, agitation, and suicidal thoughts (class-wide antiepileptic warning).
  • DRESS / Multiorgan Hypersensitivity: Drug Reaction with Eosinophilia and Systemic Symptoms characterized by fever, rash, lymphadenopathy, and organ involvement (hepatitis, nephritis).
  • Anaphylaxis and Angioedema: Swelling of the lips, tongue, throat, and acute airway compromise.

Precautions & Drug Interactions

  • Opioids and CNS Depressants: Combining gabapentin with opioids (e.g., oxycodone, morphine, fentanyl), benzodiazepines, or alcohol causes additive central nervous system depression, substantially increasing the risk of fatal respiratory failure and profound sedation.
  • Antacids (Aluminum/Magnesium Hydroxide): Co-administration with magnesium- or aluminum-containing antacids reduces gabapentin bioavailability by approximately 20%. Gabapentin should be taken at least 2 hours after antacid administration.
  • Abrupt Discontinuation: Discontinuing gabapentin abruptly can precipitate rebound neuropathic pain, severe anxiety, insomnia, nausea, and in patients with epilepsy, status epilepticus or increased seizure frequency. Doses should be tapered gradually over at least 1 week under medical supervision.
  • Driving and Operating Machinery: Patients should avoid driving until they know how gabapentin affects their alertness and psychomotor coordination.

Safety Warnings

Critical Clinical Warnings:

  • Life-Threatening Respiratory Depression: The FDA issued drug safety communications warning of serious breathing difficulties when gabapentinoids are used in elderly patients, individuals with COPD or sleep apnea, or when co-prescribed with opioid analgesics.
  • Suicidality Monitoring: Antiepileptic drugs increase the risk of suicidal thoughts or behavior; patients starting or titrating gabapentin should be closely monitored for emergence or worsening of depression, unusual mood changes, or self-harm ideation.
  • Dependence and Withdrawal: Physical dependence can develop with sustained therapeutic use; sudden cessation can trigger withdrawal symptoms similar to benzodiazepine or alcohol withdrawal. >
