Mechanism of Action (Pharmacology)

Cetirizine is a major active carboxylated metabolite of hydroxyzine and operates as a selective peripheral antihistamine:

  • Selective Peripheral Receptor Antagonism: Cetirizine competitively inhibits histamine at peripheral receptor sites on effector cells. By preventing endogenous histamine from binding, it suppresses the allergic cascade – blocking vascular permeability, capillary dilation, smooth muscle contraction, and sensory nerve stimulation.
  • Anti-Inflammatory & Late-Phase Inhibition: Beyond receptor blockade, cetirizine inhibits late-phase allergic mediator release, notably reducing eosinophil chemotaxis and recruitment to allergic inflammation sites (nasal mucosa and skin).
  • Limited Blood-Brain Barrier Penetration: Because it is amphoteric and carries a polar carboxylic acid side chain, cetirizine exhibits low lipid solubility and limited penetration through the blood-brain barrier compared to first-generation antihistamines (e.g., diphenhydramine). This significantly reduces – though does not entirely eliminate -central sedation.

Therapeutic Indications & Dosage

Approved Uses

  • Allergic Rhinitis: Treatment of seasonal (hay fever) and perennial allergic rhinitis symptoms, including sneezing, rhinorrhea (runny nose), post-nasal drip, nasal pruritus, and ocular itching/tearing.
  • Chronic Urticaria (Hives): Relief of pruritus, reduction in the number of hives, and management of chronic idiopathic urticaria and allergic skin conditions.

Dosage Guidelines

  • Adults & Children (12 years and older):
  • 10 mg once daily (one whole tablet).
  • For milder symptoms, a starting dose of 5 mg once daily may be sufficient.
  • Maximum dose: 10 mg in a 24-hour period.
  • Children (6 to 11 years): 5 mg to 10 mg once daily, depending on symptom severity.
  • Renal & Hepatic Impairment: In patients with moderate renal impairment (eGFR 30–50 mL/min) or hepatic impairment, the recommended dose is reduced to 5 mg once daily due to prolonged clearance. It is contraindicated in end-stage renal disease.
  • Administration: Can be taken with or without food. If drowsiness occurs, taking the dose in the evening before bed is recommended.

Rare / Serious Adverse Reactions

  • Urinary Retention: Difficulty urinating or incomplete bladder emptying, primarily in patients with pre-existing prostatic hypertrophy.
  • Paradoxical CNS Stimulation: Agitation, restlessness, insomnia, or hallucinations (more common in young pediatric patients).
  • Severe Rebound Pruritus: Intense, widespread itching reported in rare cases following the sudden discontinuation of long-term cetirizine use.

Precautions & Drug Interactions

  • CNS Depressants & Alcohol: Concomitant use with alcohol, benzodiazepines, sedatives, or opioid analgesics produces additive central nervous system depression, severely impairing motor coordination and alertness.
  • Driving & Heavy Machinery: Although classified as a non-sedating/low-sedating antihistamine, cetirizine causes dose-dependent drowsiness. Patients should evaluate their personal response before driving or operating heavy equipment.
  • Anticholinergic Conditions: Exercise caution in patients with prostatic hyperplasia, urinary retention, or narrow-angle glaucoma.
  • Theophylline Interaction: High-dose theophylline slightly reduces the clearance of cetirizine, potentially raising systemic exposure.
  • Allergy Skin Testing: Antihistamines suppress histamine-induced skin reactivity; cetirizine should be discontinued at least 3 to 4 days prior to allergen skin prick testing to prevent false-negative results.

Warnings & Safety Guidelines

  • Pregnancy & Lactation: Cetirizine is categorized as FDA Pregnancy Category B (widely considered a preferred antihistamine in pregnancy, though medical consultation is advised). It is excreted in human breast milk in low concentrations and is generally avoided during breastfeeding when alternative low-excretion agents are available.
  • Overdose Risk: Symptoms of overdose typically present as extreme somnolence in adults, or initial agitation and restlessness followed by drowsiness in children. Overdose management is primarily supportive and symptomatic.
