Mechanism of Action

Meloxicam belongs to the enolic acid class of NSAIDs. Its anti-inflammatory, analgesic (pain-relieving), and antipyretic (fever-reducing) effects stem from its ability to inhibit the enzyme cyclooxygenase (COX):

  • COX-2 Selectivity: At therapeutic doses, meloxicam exhibits preferential inhibition of COX-2 over COX-1. COX-2 is responsible for producing inflammatory prostaglandins (chemical messengers that cause pain and swelling at joint tissues).
  • COX-1 Preservation: By sparing COX-1 enzymes – which regulate gastrointestinal mucosal protection and platelet aggregation – meloxicam lowers (but does not eliminate) the incidence of severe stomach irritation compared to non-selective NSAIDs like ibuprofen or naproxen.
  • Pharmacokinetics: Meloxicam is slowly absorbed after oral administration, reaching peak plasma concentration in 5–6 hours. It possesses an extended elimination half-life of approximately 15 to 20 hours, allowing for once-daily dosing.

Indications & Standard Dosage

Primary Indications (FDA-Approved)

  • Osteoarthritis (OA): Symptomatic management of chronic joint pain and inflammation.
  • Rheumatoid Arthritis (RA): Relief of chronic systemic joint swelling, tenderness, and morning stiffness.
  • Juvenile Idiopathic Arthritis (JIA): Approved for pediatric patients weighing (132 lbs).

Dosing Guidelines

  • Adult Osteoarthritis & Rheumatoid Arthritis:
  • Starting Dose: 7.5 mg orally once daily.
  • Maximum Recommended Dose: 15 mg (one CIPLA 159 tablet) orally once daily. Exceeding 15 mg daily does not provide increased efficacy and significantly heightens toxicity risks.
  • Pediatric JIA : 7.5 mg to 15 mg once daily.
  • Administration: Should be taken with water, preferably with meals or milk, to minimize potential stomach upset.

Side Effects

Common Side Effects

Most adverse effects are gastrointestinal or systemic in nature:

  • Digestive System: Heartburn, abdominal pain, diarrhea, flatulence, nausea, or indigestion.
  • Nervous System: Dizziness, headache, or mild drowsiness.
  • Dermatologic / Systemic: Fluid retention (edema), mild skin rash, or minor elevation in blood pressure.

Severe Adverse Effects (Seek Immediate Medical Care)

  • Gastrointestinal Bleeding / Ulceration: Black/tarry stools, vomiting material that resembles coffee grounds, or severe stomach pain.
  • Cardiovascular Events: Chest pain, shortness of breath, sudden weakness or numbness on one side of the body, or slurred speech.
  • Hepatotoxicity: Jaundice (yellowing of skin/eyes), dark urine, upper right abdomen pain, or severe fatigue.
  • Nephrotoxicity / Kidney Injury: Sudden reduction in urine output, swelling in legs/feet, or rapid weight gain.
  • Severe Skin Reactions: Stevens-Johnson Syndrome (SJS) or Toxic Epidermal Necrolysis (TEN) presenting as flu-like symptoms followed by a painful, spreading rash and blistering.

Boxed Warnings & Precautions

Like all prescription NSAIDs, meloxicam carries FDA Black Box Warnings:

Cardiovascular Risk

  • NSAIDs cause an increased risk of serious cardiovascular thrombotic events, including myocardial infarction (heart attack) and stroke, which can be fatal.
  • Contraindication: CIPLA 159 is strictly contraindicated for perioperative pain management in patients undergoing Coronary Artery Bypass Graft (CABG) surgery.

Gastrointestinal Risk

  • NSAIDs cause an increased risk of severe gastrointestinal (GI) adverse events including bleeding, ulceration, and perforation of the stomach or intestines, which can occur at any time without warning and may be fatal. Elderly patients are at higher risk.

Pregnancy Warnings

  • Avoid from 20 Weeks Onward: Taking meloxicam at 20 weeks or later in pregnancy can cause fetal renal dysfunction leading to oligohydramnios (low amniotic fluid) and premature closure of the ductus arteriosus in the fetal heart.

  • Alcohol – Ethanol – Exacerbates stomach lining irritation and bleeding risk.
