Mechanism of Action

Acetaminophen acts primarily on the central nervous system (CNS) to manage pain and body temperature through two main pathways:

  • Pain Relief (Analgesia): It is believed to inhibit cyclooxygenase (COX) enzymes in the brain, reducing the production of prostaglandins—the chemical messengers responsible for signaling pain.
  • Fever Reduction (Antipyresis): It acts directly on the heat-regulating center of the hypothalamus in the brain, causing blood vessels to dilate and increasing sweating, which dissipates excess body heat.
  • Note on Extended-Release: The L544 caplet is bi-layered. The first layer dissolves quickly to provide rapid pain relief, while the second layer releases the remaining active ingredient slowly over an 8-hour period to sustain that relief.

Indications & Dosage

This medication is indicated for the temporary relief of minor aches and pains associated with muscle strains, backaches, the common cold, headaches, toothaches, menstrual cramps, and minor arthritis pain.

Standard Adult Dosage (Ages 12 and Over)

  • Dose: Take 2 caplets every 8 hours with water as needed.
  • Administration: Swallow the caplets whole. Do not crush, chew, split, or dissolve them. Doing so breaks the extended-release mechanism and releases all 1,300 mg of the medication at once, increasing the risk of toxicity.
  • Maximum Daily Limit: Do not exceed 6 caplets (3,900 mg) within any 24-hour period.
  • Duration: Do not take this medication for more than 10 consecutive days for pain, or 3 days for a fever, unless explicitly directed by a healthcare professional.
  • Pediatric Use: Do not use this 650 mg extended-release formulation for children under 12 years of age.

Side Effects

While acetaminophen is generally safe when taken exactly as directed, some individuals may experience side effects.

Rare but Severe Side Effects

  • Alcohol Interaction: Do not consume 3 or more alcoholic drinks daily while using this medication, as chronic alcohol use significantly multiplies the risk of severe hepatotoxicity (liver damage).
  • Pre-existing Conditions: Speak with a doctor before use if you have a history of liver disease, severe kidney disease, or are suffering from severe malnutrition or dehydration.
  • Drug Interactions: Consult a doctor or pharmacist prior to use if you are taking the blood-thinning medication warfarin, as regular acetaminophen use can increase your risk of bleeding.
